He uses your own fat, taken from areas where you don’t want it, and then re-injects it into your buttock areas where you do want it. The actual fat extraction is achieved with liposuction. After purification of the extracted fat, the best quality fat (where the fat cells are more likely to survive after re-injection) then gets threaded back into all layers of the buttocks by a method called fat grafting.
